Physical and mental health are fundamentally linked. Nowhere is this more evident a problem than in the area of chronic conditions. Poor mental health and mental illness are risk factors for chronic physical conditions, such as diabetes and heart disease. At the same time, people living with chronic physical conditions often experience poor mental health.
